Engine

The V6 in a lot of MDX years is known for a VTC actuator rattle, a quick grind on a cold start that owners hear and worry about. Some of these engines use a bit of oil as the miles climb, so a check between changes keeps you from running low. The direct-injection ones cake carbon onto the intake valves over time, which turns into a stumble. Gaskets harden and seep, leaving a spot on the driveway. A tired coil or plug throws a misfire and lights the dash.
Cooling System
COMMON PROBLEMS: Water Pump Leaks, Coolant Loss, Thermostat Failure, Overheating, Coolant Smell
The MDX cooling system holds up, but heat wears it down over the years. The water pump weeps at the gasket as it ages, and hoses and the radiator start leaking at the seams. A stuck thermostat sends the temperature the wrong way, too hot or never quite warming up. Half the time you smell the coolant before you ever see a drip. Let a slow leak run and it turns into an overheat, and on a V6 this size that's a far bigger bill than a hose or a pump.
Fuel System

It feels like an ignition problem, which is exactly why guessing gets expensive.
Electrical System

There's a lot of electronics packed into an MDX, so there are more places for a gremlin to hide. A battery dead in the driveway from a drain you can't see. Suspension

The MDX rides planted and quiet when the front end is fresh, so you notice when that changes. Struts go slow, and the giveaway is a body that bounces or floats over dips instead of settling. Control arm bushings and sway bar links wear too. They knock over bumps, or turning into the driveway. Worn parts drag the alignment off, so the tires wear uneven and the wheel picks up a shimmy at speed. On a heavy SUV, catching it early keeps a small fix from becoming a tire bill on top.
